Introduction : Les facteurs favorisant la survenue de la gonarthrose ont été très peu étudiés en Afrique subsaharienne. Objectifs : Déterminer la fréquence des facteurs de risque de la gonarthrose au cours d'une consultation rhumatologique à Lomé (Togo). Patients et méthodes : Etude rétrospective sur dossiers réalisée sur une période de 17 ans chez des patients vus en consultation de rhumatologie au CHU-SO de Lomé.

Résultats : Des 13963 consultants de rhumatologie, 1080 (8%, 907 femmes et 173 hommes) souffraient d'une gonarthrose répondant aux critères de l'ACR. L'âge moyen au début de la maladie était de 50,9±11,3 ans (extrêmes : 17 et 85 ans) et la durée moyenne d'évolution de 4,2±5,7 ans. L'atteinte était unilatérale chez 518 patients (48%) et bilatérale chez les 562 autres (52%). La symptomatologie était dominée par une douleur d'horaire mécanique (1015 patients, 94%) et une réduction du périmètre de marche (292 patients, 27%). L'arthrose était essentiellement fémorotibiale interne (538 patients, 49,8%). Aucun facteur de risque n'a été retrouvé chez 38 patients (3,5%). Le sexe féminin (907 patients, 84%), la surcharge pondérale et l'obésité (837 patients, 77,5%), et la déformation en varus et/ou en valgus (799 patients, 74 %) ont été les principaux facteurs de risque retrouvés. Les trois facteurs étaient simultanément présents chez 49,3 % des patients. Des 907 femmes souffrantes de gonarthrose, 83,6% étaient obèses et 64% des femmes obèses avaient une déformation en varus et/ou en valgus.

Conclusion : Le sexe féminin, l'obésité et les déformations en valgus et en varus constituent les principaux facteurs de risque d'arthrose fémorotibiale en Afrique Noire.

Introduction : Risk factors of osteoarthritis of the knee have not been sufficiently studied in the Sub-Saharan Africa. Aim of the work : To determine the frequency of the risk factor of osteoarthritis of knee in patients admitted to the Rheumatology Department in Lomé (Togo). Patients and Methods : A retrospective study was conducted over 17 years on records of hospital patients admitted for Osteoarthritis of the knee Medical records of patients admitted to the Rheumatology Department in Lomé (Togo).

Results : Among the 13963 patients seen, 1080 (8%, 907 women and 173 men) had osteoarthritis of the knee responding to ACR criteria. Mean age at onset was 50.9±11.3 years (extreme: 17 and 85 years), and mean duration of osteoarthritis of the knee was 4.2±5.7 years. The osteoarthritis affected one joint in 518 patients (48%) and two joints in 562 others patients (52%). Mechanic pain (1015 patients, 94%) and claudication 292 patients, 27%) were the frequent symptoms. At the knee, the lesions involved the medial femorotibial compartment in 538 patients, the lateral femorotibial compartment in 242 patients, the patellofemoral compartment in 46 patients, and the femorotibial and patellofemoral compartment in 255 patients. Only 38 patients (3.5%) had no identifiable risk factors. 1042 patients (96.5%) had at least one of these risk factors. The main risk factors in the remaining patients were Female sex (907, 84%), Obesity (837 patients, 77.5%), and varus or valgus deformities (799 patients, 74%). 151 patients (14%) had one of these risk factors, and 532 (49.3%) had all three. Among 907 females with osteoarthritis of knee, 83.6% were obese and 64% of females obese had varus and/or valgus deformities.

Conclusion : Female sex, obesity, and varus or valgus deformities are the main risk factors of the knee osteoarthritis in black Africa.
